SMART Heart Meal Program Receives $3,000 Grant

Concord Hospital received a $3,000 grant from the Bishop’s Charitable Assistance Fund to support its SMART Heart Meal Program. The program supplies discharged patients with three days of low-sodium meals along with a 30-day supply of meals delivered at no cost from Meals on Wheels. The program's ultimate goal is to reduce the number of patient readmissions to the hospital within 30 days of discharge.
